Pre-Appointment Screening

The below questionnaires have been developed to help us determine how best to guide your care. The information you provide us will tell us if you are eligible for specific services under the BC Medical Services Plan (MSP).

Once you have completed your questionnaire, click on the submit button, this will send your responses to be uploaded into your medical file. If you are eligible for a specific service we will then contact to to set up an appointment.
